In aftercare, we provide support to children and young people following the end of their care or placement. Typically, aftercare begins once a placement has concluded, though it may start later. Young people are entitled to aftercare (in accordance with the Child Welfare Act) for five years after their last child welfare service or until they reach 23 years of age. Before the 2024 amendment, this age limit was 25.
However, those aged 23-24 will continue to receive services from the aftercare team until they turn 25 (services in accordance with the Social Welfare Act). They may be eligible for financial assistance and possibly supported housing.
Our aftercare services offer support with housing, finances, health, education, employment, and independent living, among other matters. A designated social worker collaborates with each young person to assess their needs and create a client plan. A social counsellor may also work with the young person to provide additional support.
We also welcome the participation of close relatives and support networks from, for instance, school or health care services.
Young people may contact us independently to start aftercare services. Once aftercare concludes, our social worker can refer the young person to other municipal services.
